2013 REGISTRATION DOCUMENTATION:
1) Pop Warner Sports Physical dated after January 1, 2013.
2) Signed player contract
3) 3 copies of year end 2012/2013 complete report card (with all four semesters) *also email to dillard@scyfbulldogs.com and dayrivette@scyfbulldogs.com if possible
4) Copy of birth certificate (new participants only)
5) Recent picture of your child (new participants and cheer only)
